I received an explanation of benefits saying that the maximum a provider could bill me directly was $0.00; and yet the provider was billing me for these two items anyway. I talked to 3 separate "customer service agents" who tried to tell me that the EOB I had downloaded from Medicare's own website must have been a web error. It turns out, they weren't looking at the same information I was - they were reading from a different column. I never did get help from 3 incredibly bored-sounding wastes of flesh. I think we do need a single payer health system in america, but the worthless dunces staffing Medicare have to go.
