This has been the most frustrating insurance experience I've ever had. They seem to intentionally complicate the claims process. Over the past 10 months, while trying to claim a pre-approved orthodontic procedure, I've had to call multiple times just to ensure my claim is processed correctly. They frequently misplace claims, assert that receipts weren't submitted (despite my documented proof), or deny my claims citing internal errors. It's been a complete nightmare. I truly wish my employer had selected a different provider.
