They charged me $50 and didn't send the forms until late on December 19th, which my employer wouldn’t accept due to errors. I had to wait until the next week to address this, but it seems my request got lost in the holiday backlog. Now, I’m appealing my denied leave due to their delays. This outsourcing of paperwork is concerning, especially when people's livelihoods are at stake.
