Was charged 3 times for a home I didn't live in

We moved home in July and wanted to stay with 4th Utility, so I rang the customer number to explain. The salesman said he could sort everything out and even gave us a better price at the new flat, including 1 month free. I was very happy with the service. However a few weeks later, I was charged for for my previous flat as well as the new home. I called again and explained the situation. The gentleman understood, apologised, and said he would refund the money. More weeks passed, no refund was issued and then I was charged again for my old flat. Once again, I phoned 4th utility to explain. The gentleman couldn't help me but said the refund was pending. After almost a month, no refund appeared. I then rang up once more and managed to speak to someone who could help. He issued the refunds as credit and ensured my previous contract was fully cancelled. I thought that had drawn a line under it, then two days later, I was charged again for my previous address. Plus, I've now received an email requesting i return the old router, and if not, I'll be charged. Its a shame, because had i received this email when we moved, I still would have had it.
