This company is a scam!! You have to give them your credit card info before you complete the questionnaire which may eliminate you from treatment, but they still have your card information and charge you. You can't reach a real person. When I emailed asking for a refund, I got a response instructing me to finish the questionnaire. I'm going to have go through my Discover card to cancel the charge.
